Phyllis Ann Smith, 62 of Dayton, Ohio passed away on September 19, 2016. She was preceded in death by her parents, Cleon Lewis & Rosie Lewis & a host of nieces & nephews. Phyllis Ann is survived by her husband, Charles L. Smith whom she married February 12, 1984; brother in law, Seldon Smith & Deanna Jones, her very good friend, brothers, Johnnie Lewis, Donald Lewis, Mike Lewis & Randa, sisters, Angela Purvis & Kenneth, Christina Calvert & Robert, a host of nieces, nephews.

FUNERAL: Friday, September 23, 2016 @ 7:00 P.M. at the Newcomer Funeral Home & Crematory-North Chapel, Dayton, Ohio.
INTERMENT: Saturday, 1:00 P.M. September 24, 2016 @ the Lewis Family Cemetery, Thousandsticks, KY.
VISITATION: Friday, September 23, @ 4:00 P.M. at the Newcomer Funeral Home.
