The obituary of Hazel Perry Officiating Minister Rev. Billy Joe Lewis and Rev. Don Kilburn Hazel Perry born November 29, 1927 departed this life on March 17, 2008 at the age of 80 years, 3 months and 17 days old. She was born at Harlan, Kentucky the daughter of the late John Turner and the late Maude Adams Turner. By occupation Hazel was a retired worker from the Bag Factory, restaurant waitress and motel employee. She was affiliated with the Full Gospel Church of Jesus Christ at Smilax, Kentucky and enjoyed gardening and going to church. Hazel was preceded in death by her parents: John & Maude Turner, she leaves surviving the following relatives: Son: Arville Perry of London, KY; Daughter: Charlean Middleton; One Brother: Jack Turner of Flemingsburg, KY.; Two Sisters: Daisy Gross of Yeaddiss, KY.; Lucille Griffith Grandchildren: Kimberly Sizemore, Katlyn Sizemore & Arville Perry, Jr.; Special Friend: Mary B. Jones; Nieces: Valerie Bowman, Hazel Brown, Carolyn Shepherd; Nephews: Vance Turner, Victor Turner, Giles Joseph, Wayne Boggs, Matthew Turner & Chase Turner. Also survived by a host of other relatives and friends. Funeral services for Hazel Perry will be held at 11:00 a.m. Friday - March 21, 2008 at the Dwayne Walker Funeral Home Chapel, Hyden, Kentucky with Rev. Billy Joe Lewis and Rev. Don Kilburn officiating. Visitation will be held from 6:00 p.m. - 9:00 p.m. Thursday March 20, 2008 at the funeral home chapel. Burial will be at the Full Gospel Church of Jesus Christ Cemetery located at Smilax, Kentucky under the direction of the funeral home staff.
