The Obituary For: Ms. Ashley Sams The Officiating Ministers: Denver Brock, Todd Roberts & William Lewis Ms. Ashley Dawn Sams born January 4, 1988 departed this life on Thursday, January 5, 2012 at the Pikeville Medical Center, Pikeville, Ky. Ashley was born in Knoxville, Tennessee the daughter of Leslie Sams & Covey Lewis. Ashley was a life-long resident of Leslie County and affiliated with the New Hope Community Church, Warbranch, Ky. Ashley was an animal lover, enjoyed reading, & being involved with children. Ashley Dawn Sams was preceded in death by her niece: Makynli, grandfather: Bill Cornett, aunt: Carol Ann Cornett May, & uncle: William "Pup" Cornett. Ashley Dawn Sams leaves the following relatives surviving: loving and devoted mother: Leslie Sams of Hyden, Ky., father: Covey Lewis, very special friend in her heart: Mikel Morgan of Essie, Ky., maternal grandparents: Donya Cornett Sizemore and husband, Roger of Hyden, Ky., Paternal Grandmother: Karen Lewis of Corbin, Ky., maternal great-grandmother: Grace Cornett of Hyden, Ky., two sisters: Amber Lewis & Emily Lewis of Corbin, Ky., treasured niece & nephew: Kendallyn Lewis and Bryson Lewis, aunts: Tina Garrison & husband, Greg of Stinnett, Ky., Ruth Collins and husband, Jackson, of Hyden, Ky., Judy Whitehead of Lexington, Ky., two very special children in her life: Madalyn Whitehead & Wadon Mikel Whitehead, very special friend: David Lewis& a host of other treasured relatives and friends. The family would like to take this opportunity to express their heart-felt & sincere appreciation to Dr. Reta & the staff at the Albert Chandler University of Kentucky Medical Center.Ashley had heart surgery performed three times at this facility. The funeral service for Ms. Ashley Dawn Sams will be conducted at 3:00 P.M. -Monday January 09, 2012 at the Dwayne Walker Funeral Home Chapel with Rev. Denver Brock, Rev. William Lewis, & Todd Roberts officiating. Entombment will follow at the Cornett Family Cemetery at Hyden, Ky. Friends may call for visitation at 5:00 p.m. - Sunday- January 8, 2012 at the Dwayne Walker Funeral Home.
